• ​To facilitate the referral process, please fax the patient's referral, recent laboratory results, and any relevant medical records to FAX 678-780-4313.

 

 

Receiving this clinical information before the initial appointment enables our Registered Dietitians to perform a thorough nutritional assessment, develop individualized medical nutrition therapy recommendations, and coordinate care with your treatment plan.

We are committed to providing timely, patient-centered care and keeping referring providers informed through consultation notes and ongoing communication, as appropriate.

• MEDICARE,  HMO Plans and some commercial plans require MEDICAL REFERRAL.

MEDICARE covers Medical Nutrition Therapy (MNT) for diabetes or chronic kidney disease (GFR 13-50, and Stage 5, not on dialysis) and 36 months post kidney transplant when a Medicare beneficiary has been referred by a physician.​

MNT provided by a Registered Dietitian (RD) may result in improved diabetes and renal disease management and other health outcomes and may help delay disease progression.
